Medication Therapy Management by Board Certified Pharmacist include:

Review all medications including supplements, herbals and other health products.

Screen for side effects/adverse effects, unnecessary medications, interactions among medications, supplements, other health products, medical conditions you may have and foods/drinks you might be taking.

Ensure that medications are appropriate, there is no overdosing or under dosing of medications.

Identify duplication of therapy, unnecessary therapy or additional therapy needed.

Establish a treatment goal based on evidence based clinical practice guidelines that is specific for patient.

Incorporate a wellness plan into treatment plan.

Communicate and coordinate with patient’s medical providers.
