­­­Hyperlipidemia Management

Background:

❱ 73.5 million US adults have high low-density lipoprotein cholesterol (LDL-C).

  • 29.5% have the condition under control.

  • <50% are getting treatment to lower cholesterol levels.

❱ Nearly 1 in 3 American adults has high cholesterol.

❱ High total cholesterol increases risk for heart disease and stroke, two leading causes of death.

❱ 33% of U. S adults with high low-density lipoprotein (LDL) cholesterol did not have it controlled at recommended levels. Because of the aging of the US population, the prevalence of all cardiovascular disease (CVD) is projected to increase to 40.5% by 2030, and the total economic burden of CVD is estimated to exceed $1 trillion annually.

❱ Adherence rates range from 25% to 40% among older adults. Cost-related medication nonadherence is a serious problem in the United States.

Our board Certified Clinical Pharmacist Practitioners in collaboration with physicians and other medical providers -

  • Perform comprehensive medication therapy management and a systematic approach to optimize patient outcomes and reduce costs.

  • Evaluate and manage chronic disease states based on clinical practice guidelines.

  • Recommend and individualize disease state goals based on patient’s need.

  • Will employ motivational techniques to tailor programs for smoking cessation, lifestyle and behavior changes, and educating groups about disease risk reduction.

  • Formulate patient‐centered treatment plans for complex patients and self-engagement.

GOAL – Hyperlipidemia
  • Total Cholesterol <199 mg/dl

  • LDL <130 mg/dl

  • HDL>50mg/dl (male); HDL>40mg/dl (female)

  • Triglyceride<149mg/dl

Eat heart-healthy foods.

Do not smoke.

Decrease the total amount of fat you eat.

Replace unhealthy fats with healthy fats.

Include fish in your diet.

Eat fruits and vegetables every day.

Eat foods high in fiber.

Limit alcohol.

Be Physically active for at least 30 minutes/5-days a week.

Maintain a healthy weight and waist size.
